USE OF PROPHYLACTIC ANTICONVULSANTS IN SUBARACHNOID HEMORRHAGE: IS THERE A PATTERN?

Aneurysmal subarachnoid hemorrhage (SAH) is considered an important cause of death and neurological sequelae. The mortality rate of this disease can reach 50% in the first two months after brain aneurysm bleeding. Despite scientific advances, the outcome of treatment has not changed in recent years. Anticonvulsant drugs are routinely used for prophylactic treatment in patients suffering from SAH. According to the research, administering anticonvulsants before surgery in low-risk patients, in the short term, because their continuous use has side effects, demonstrates effectiveness. However, there is not enough evidence to justify its use in the primary and secondary prevention of seizures after subarachnoid hemorrhage. The pathophysiological heterogeneity of patients affected by SAH, the absence of satisfactory pharmacokinetic investigations to assess optimal doses, and the timing of administration reinforce the need for further studies to design a preclinical therapeutic model.
